All new clients must fill out Intake documents, HIPPA release, and consent forms. These forms will be emailed to the client immediately after booking an initial appointment. Please complete forms and submit the forms prior to the upcoming appointment.
INTAKE POLICY: All Intake forms, consents and insurance benefit verification must be received a minimum of 72 hours prior to initial appointments or appointment will be rescheduled.
You will receive an email and text reminder, numerous times prior to your scheduled appointment. Kindly confirm attendance either via text or email.
In the event that you need to cancel or re-book your appointment, we ask that you provide us with a full 24 hour notice. Last minute cancellations will result in a $50 fee.
Please note that all late arrivals will result in shortened services in order to properly serve all our other clients. Our treatments are booked back to back. If you are more than 15 minutes late, we may not be able to treat you. Some late arrivals may be charged a $50 cancellation fee.
Our practice maintains client confidentiality in-accordance with the Health Insurance Portability and Accountability Act (HIPAA) and the U.S. Department of Health and Human Services (HHS) Office for Civil Rights (OCR). Our trained staff preserves, protects and maintains confidentiality to ensure that our client's feel safe while being cared for at our practice.
We safeguard our practice with secure HIPAA compliant electronic health care record platforms, Tebra and Aesthetic Record.
All information a client reports during psychotherapy sessions stays in private, this is called a "privileged relationship." Privileged communication is a legal term that refers to information exchanged between two parties that is protected by the law and cannot be forced to be disclosed.
However, there are times when a there is a legal obligation to break confidentiality, as follows:
Prior to beginning services, all clients are required to provide consent to HIPPA, privacy policy, authorization to receive care and treatment. All Intake and consent forms are sent via our secure EHR portal systems.
Our practice requires clients to be seen a minimum of every 90 days for on-going psychotropic medication prescribing. This allows our provider to assess a client's well-being, monitor for side effects, and make any necessary adjustments to the medication regimen.

For safety reasons and insurance purposes, NO CHILDREN UNDER THE AGE OF 12 are permitted in the aesthetic or clinical treatment areas. We do not have the facilities to care for children or pets, and hope you understand we cannot assure their safety in a professional environment.
As a form of assessment and documentation, our practice (during pre/post procedures) utilize photography. Our practice stores and maintains photographs in a secure medical chart, a HIPPA compliant EMR. Clients are asked to consent, authorize or deny the use of photography pertaining to their images. Consents include permission to use photography as follows: medical documentation, educational/training purposes , website, and social media. All clients have the right to deny the use and/or posting of photography.
All services and treatments are FINAL SALE. All products purchased from are considered FINAL SALE. They are non-returnable and non-refundable.
Regardless of the service a client is requesting, the first point of contact with our practice is requesting a Complementary Consulation or Appointment. To request these services, clients will find scheduling buttons in multiple areas within our website, under each specific service tab and under the Appointments tab. Appointment request allows our practice to confirm staff availability and to gather basic information and a very brief description of what services are being requested. Do your best to describe current issue(s). We will evaluate specific requests, conditions, and symptoms as care begins and progresses.
We are a "Therapy Animal Friendly Practice," meaning that at any given psychotherapy appointment a therapy animal may be present at the practice. The animals are present based on their willingness of the day. If they are not up to the work task for the day, for many reasons, they get a no fault pass for work. They are the bosses (after all!). Regardless as to if a client would like a Practice Therapy Animal in their session, all clients are required to completed an Animal-Assisted Therapy Consent (this is within Intake Forms) to enter the practice building. Our Practice Therapy Animals are only introduced to clients who desire the comfort of the animal. All of our Practice Therapy Animals have engaging, predictable temperaments, are obedience trained, and have received Animal-Assisted Therapy Training and Certification.
